THE EFFECT OF VITAMIN D SUPPLEMENTATION ON CLINICAL SYMPTOMS, ANXIETY, DEPRESSION AND SERUM LEVELS OF CRH AND IL-6 IN PATIENTS WITH DIARRHEA-PREDOMINANT IRRITABLE BOWEL SYNDROME

Background and Aim: Vitamin D deficiency is common in the population with IBS which can increase depression, anxiety and low-grade inflammation in these patients. These factors lead to clinical symptoms of the disease. Corticotropin-releasing hormone (CRH) is secreted in response to stress from the paraventricular of the hypothalamus. CRH increases in patients with IBS-D. In addition, elevated in IL-6 have been seen in these patients. Those factors lead to raise in the intestinal movement, absorption, secretion, permeability, and immune system activity that can cause diarrhea in IBS subjects. Consequently, we designed this study for assessment effect of vitamin D supplementation in diarrhea-predominant IBS patients.Methods: 88 IBS patients (age: 18-65) participated in a randomized, placebo-controlled trial study for 12 weeks. Subjects were randomly divided into each group that received weekly 50000 IU vitamin D3 or placebo. IBS severity score system (IBS-SSS), IBS-quality of life questionnaire (QoL), Hospital Anxiety and Depression Scale (HADs), visceral sensitivity index (VSI) and serum 25(OH) vitamin D3, CRH and IL-6 were evaluated before and after interventions.Results: After 12 weeks intervention with vitamin D3, patients showed significant improvement total IBS-SSS (P 0.0001), total QoL score (P=0.049), HADs-depression (P=0.023), VSI score (P=0.008), serum CRH (P=0.05), and serum IL-6 (P= 0.027) as compared to placebo group. There were no significant differences in change HADs-stress scores between the two groups. Conclusion: Our results indicated that vitamin D supplementation can improve IBS symptoms, Quality of Life, depression, visceral sensitivity related to anxiety, serum CRH and IL-6 in IBS-D patients.
