Evaluation of the Association between Maternal High Blood Pressure, Heart Disease and Gestational Diabetes and Neonatal Mortality :A case-control Study

Background and Aim: Prevention of non-communicable diseases such as Diabetes Mellitus (DM), hypertension and kidney diseases during pregnancy is recommended due to their serious complications for mothers and infants. Considering that these diseases have a great impact on infant mortality, this study was conducted to investigate the relationship between mothers’ underlying diseases and gestational diabetes and infant mortality in Iran. Materials and Methods: This case-control study used data collected from pregnant women who referred to health centers in 9 provinces of Iran. There were 1162 cases and 1624 controls. The required data was collected from mothers’ health records and through interviews. Logistic regression was used to analyze the results. Software SPSS 19 was also used for statistical analysis while significance level was less than 0.05. Results: The chance of neonatal mortality among mothers with high blood pressure (hypertension) was 3 times higher compared with healthy mothers (OR= 3.04; 95% CI: 4.657-1.985, P <0.001). In terms of gestational diabetes, mothers who had not undergone any gestational diabetes tests had a 3.59 times higher chance of neonatal mortality compared with the mothers without gestational diabetes (OR = 3.59, CI: 2.72-4.75, P = 0.0001). The chance of neonatal mortality in women with kidney disease was also 1.64 times higher than mothers without kidney problems (OR = 1.64, CI: 1.1-2.45, P = 0.015). Conclusion: This study showed that undiagnosed underlying diseases such as hypertension and diabetes during pregnancy were related to neonatal mortality, which highlights the importance of caring for and counseling about the underlying diseases, screening and controlling blood sugar level before and during pregnancy in order to prevent infant mortality by all means possible.
