چکیده
Background: Melanoma is an increasingly important public health problem worldwide. the incidence of melanoma has been increasing faster than that of any other cancer in the world. invasive melanoma currently is the fifth most frequently diagnosed cancer in men and the sixth most frequently diagnosed cancer in women in united states. As there is still no effective curative treatment in patients with disseminated melanoma, it is important to estimate the risk for progression in the individual patient, preferably at the time point of primary diagnosis. Breslow tumor thickness is still the best prognostic factor for this issue.This study sought to determine if the use of immunohistochemical stains, specifically S-100 and HMB-45 and Melan-A immunostains, show greater tumor thickness than appreciated on hematoxylin and eosin (H&E) stained sections. Methods: The study was performed on 36 cases with invasive malignant melanoma treated in the Razi hospital between 2008 and 2010. Histologic sections were obtained from each tissue block and stained with H&E, S-100, Melan-A and HMB-45 immunostain , then the level of invasion and tumor thickness, were evaluated bydermathopathologist. Differences between Breslow depths recorded with H&E and the corresponding depths measured using immunostains were then compared using the paired 2-tailed student t-test. Results: The difference between the means of tumor thickness with H&E and S-100 was about 0.22 mm (CI: 0.099-0.348) (P=0.001) and the difference between the mean of tumor thickness with H&E and Melan-A was about 0.13 mm (CI: 0.023-0.237) (P=0.018) , but There is no significant difference between the mean of tumor thickness with H&E and HMB-45 (0.0625 mm) (CI: -0.052-0.177) (P=0.278) and There was no significant difference between the level of clark in immunohistochemical stains and hematoxylin and eosin. Conclusions: According to the significant differences between the means of tumor thickness with H&E and S-100 and H&E and Melan-A, immunostains is more precise than H&E for determining the tumor thickness.کلیدواژه ها
