Background and objectives: Carcinoma in situ (CIS), also known as in situ neoplasm, is expression of malignant epithelial cells. This flat lesion is referred to as ductal carcinoma in situ (DCIS) and lobular carcinoma in situ (LCIS). Although some forms of CIS have a high probability of progression into breast cancer, not all CIS becomes invasive. In this case the treatment is recommended by a combination of surgery, radiotherapy and hormone therapy. Histology also helps physicians to evaluate the invasive capacity of this malignancy type. The objective of this study is to analyze survival of invasive breast cancer patients considering 1) with in situ neoplasm, and 2) without in situ neoplasm, and to provide a comparative analysis. Materials and Methods: In this study all files of breast cancer patients treated in three hospitals of Tehran in the period of 2007 till mid 2016 are investigated (2010 files). Due to incompleteness of some records, it was provided that 1822 patients’ data to be finally considered. In this comprehensive study all the related information has been analyzed comprehensively. In this study tissue and the pathology type of the patients are also investigated; however most of the survival sources not reported the investigation. Results: Besides independent variables addressed by different sources of literature, this study reveals that the both medical history (such as diabetes, hypertension, and internal glands disorders such as hypo- and hyperthyroidism) and extra capsular extension play an important role in the hazard function of a patient’s survival analysis. The analysis of this study indicates that the survival rate corresponding to 1, 2… 9 years (regardless of the disease stage) for a patient with diagnosed in situ neoplasm, are more compared to someone who has not had in situ neoplasm. Conclusion: The survival analysis considering neoplasm leads one to an effectiveness model compared to the case that neoplasm is not attended. This study indicates that 1) the survival rate of breast cancer patients with in situ neoplasm (without considering disease stage) is more than one who is not initially supported by invasive carcinoma, and 2) in the case of existence of the both in situ neoplasm and invasive malignancy, after the 4th year, the life expectancy is increased compared to the one with only invasive malignant
