Infiltrative ductal carcinoma within fibroadenoma, a case report.

چکیده

Introduction: Fibroadenoma is the most common cause of breast masses in young women. Many studies have described that fibroadenomas do not have elevated risk for developing breast carcinoma,unless the tumor or the surrounding breast tissue reveals proliferative changes, or the patient has a positive family history of breast carcinoma. Rarely, lobular and ductal pre-malignant or infiltrative carcinoma may occur within fibroadenoma. We report a case of infiltrative ductal carcinoma within fibroadenoma.Case presentation: A 43-year-old woman presented with a palpable lump in her left breast for five months, at the Rajaee hospital, in September 2010.The familiy history of breast cancer was negative. On physical examination,there was a non-tender, mobile, well circumscribed mass lesion in the upper outer quadrant of the left breast.Associated axilla was unramerkable. Mammography revealed a partially well-circumscribed mass lesion with variable density. Gross examination of excised mass with safe margin shown a partially ill defined mass that was 3x2.5 cm in size. Histopathological review of mass revealed elongated compressed ducts and stromal proliferation with few foci ductal carcinoma in situ and few foci of invasive ducts and nests of malignant ductal epithelial cells in the surrounding stroma. Subsequently,the patient under went modified radical mastectomy.No tumor residue was detected. Axillary lymph node was free from tumor.The patient is free of disease or recurrences during follow-up.Conclusion: Despite the incidence of infiltrative ductal carcinoma within fibroadenoma is rare and very low , the outcome associated with accurate diagnosis.Therefore each breast mass should be evaluate, carefully.
