چکیده مقاله:

Background: At present years, risk assessment is an important tool in risk management to reduce project risks and achieve sustainable development. The aim of this study was identified, controlled and prevented hazardous that was approached to eliminate errors occurrence of asyringe factory Method and material: This study was descriptive- analysis study. This has done in one of the syringe factory in urmia city in 1395.In this study, hazards and damages identified, after observingthe workplace and interview with employees. Then several Focus Group Discussion formed with occupational health and ergonomic experiments. A questioner was designed that including;probability of failure, severity of failure and probability of detection. Finally, level of hazard prioritized for classifying hazards. The data analysis done using failure mode and effect analysisResults: 14 cases failed, 15 cases failed and 12 failed effect were fined which were related to harmful agents in workplace to identify and recognize with 90% reliability. 1 case fail effect tocause by respiratory diseases that was released via vapor from spring tinnier in print unit with equal RPN≥150.That result was identified as a unacceptable risk Conclusions: hazards identified and classified from point of employees and experiment in vari able units which show FMEA is a good tool for intervention and reduce hazards
