چکیده
Background and Objective:Type ۲ diabetes is characterized by impaired glucose metabolism, insulin resistance, and dyslipidemia. Complementary approaches, such as exercise interventions and the use of herbal extracts, have gained importance as non-pharmacological strategies for the management of this disease.This study aimed to investigate the effects of eight weeks of moderate-intensity interval training combined with neem (Azadirachta indica) extract supplementation on lipid profile, insulin resistance, and cytokeratin-۱۸ (CK-۱۸) levels in men and women with type ۲ diabetes.Methods: In this semi-experimental study, ۶۰ individuals with type ۲ diabetes were randomly assigned to one of four groups: Exercise+Supplement, Exercise+Placebo, Supplement only, and Control. The exercise groups performed moderate-intensity interval training three times per week, while the supplement groups received neem extract capsules twice daily for eight weeks. Variables assessed before and after the intervention included lipid profile (total cholesterol, LDL, HDL, triglycerides), insulin resistance (HOMA-IR), and cytokeratin-۱۸ levels. Results: After eight weeks, the Exercise+Supplement group demonstrated significant reductions in total cholesterol, LDL, triglycerides, insulin resistance, and CK-۱۸ levels, along with a significant increase in HDL levels (P< ۰.۰۵). The Exercise+Placebo group also showed significant improvements in several indices (P< ۰.۰۵), whereas no significant changes were observed in the Supplement and Control groups (P> ۰.۰۵). Conclusion:The combination of moderate-intensity interval training and neem extract supplementation appears to be an effective complementary strategy for improving lipid profile, enhancing insulin sensitivity, and reducing cellular damage in patients with type ۲ diabetes. These findings underscore the importance of multi-modal, non-pharmacological interventions in diabetes management.کلیدواژه ها
