Background & aim: COVID-۱۹ infection may adversely affect pregnancy outcomes. We aimed to assess the incidence rate of the COVID-۱۹ virus and its effect on maternaland neonatal outcomes.Methods: This was a cohort study in which a number of health-care centers in Tehran province were randomly selected, and all pregnant women referring to these centers were screened for COVID-۱۹ from April ۲۰۲۰ to July ۲۰۲۰. Out of ۱۵۵۲۰ pregnant women, ۲۶۳ individuals infected with COVID-۱۹, who were followed until delivery. To collect the data, a demographic and obstetric characteristics questionnaire and a maternal and neonatal outcome checklist were used. Data analysis was done by SPSS version ۲۲ using the Chi-square test, independent t-test, and logistic regression model.Results: The incidence of COVID-۱۹ among participants was ۱۷ per ۱۰۰۰ (۹۵% CI: ۱۵-۱۹). Women with a higher BMI (OR = ۱.۱۹۸, CI =۱.۰۰۳-۱.۴۳۱, P = ۰.۰۴۷) and lower blood oxygen level (OR= ۰.۸۸۶, CI: ۰.۸۰۸-.۹۷۰, P=.۰۰۹) were more likely to have adverse pregnancy outcomes. Preterm labor (۱۸.۳%) was the most common outcome of pregnancy, followed by stillbirth (۰.۸%), ICU admission (۳.۰%), maternal mortality (۱.۱%), need to mechanical ventilation (۱.۵%) and preeclampsia ۱(۰.۳۸). Regarding neonatal outcomes, neonatal COVID-۱۹ was seen in ۱.۹% of cases, NICU admission in ۱۵.۲%. Neonatal mortality without COVID-۱۹ in ۲.۳%.  The majority of neonates with complications were male (۲۹.۱% vs. ۲۱.۵%, (P= ۰.۰۰۲).Conclusion: Pregnant women had a lower COVID-۱۹ incidence rate than the general population, but when infected, they were at risk for poor maternal and neonatal outcomes.
