چکیده مقاله:

Hyperglycemia occurs in the diabetic and non-diabetic patients with a broad range of etiologies. The treatment outcome and the responsiveness to the insulin therapy are dependent on the underlying mechanisms of pathogenesis. As well as, hyperglycemia affects the results of other analytical tests. In this case report, we present an elderly patient with acute severe hyperglycemia without any diabetes history who exhibited a poor and refractory response to insulin therapy.An ۸۵-year-old patient received insulin and vitamins B for treatment of acute hyperglycemia. The blood glucose measurement and other biochemical tests were performed routinely. Due to the abnormal serum renal function biomarkers, kidneys were evaluated by ultrasonography. The data were analyzed by SPSS version ۲۳ and the records and finding were described and represented in detail.The patient had poor response to insulin therapy. The administration of B vitamins potentiates the efficacy of insulin. In this case, the serum creatinine and BUN levels were higher than age-adjusted normal range. The ultrasonography results showed that the kidney structure was normal which was confirmed by normal urine analysis results. In spite of the normal ultrasonography results and urinary tests, the serum creatinine and BUN levels were higher than normal range and were reduced by reduction of serum glucose levels.Based on these observations, vitamin B adjuvant therapy is recommended to the treatment of acute hyperglycemia in the elderly patients with unknown etiology. As well as, post- glucose correction renal function tests re-analysis is recommended
