Comparing The Effect of Bone Marrow MononuclearCells and Mesenchymal S tem Cells on Left VentricularFunction: A Meta-Analysis of Clinical Trials

چکیده مقاله:

Objective: Bone marrow mononuclear cells (BM-MNCs) andmesenchymal s tem cells (MSCs) are two lines of s tem cells thathave been used in the setting of acute myocardial infarction(MI) and both types have displayed clinical efficacy by improvingleft ventricular ejection fraction (LVEF). However, no s tudyhas compared these two types of cells directly.Materials and Methods: In this meta-analysis, we screenedthrough databases (PubMed, Scopus, Embase, and CENTRAL)searching for clinical trials inves tigating the impact of eitherBM-MNCs or MSCs on ventricular indices and hospitalizationrate for heart failure (HF). We conducted this meta-analysis usingS tata software version ۱۳.Results: In total, ۳۶ trials (۲۶ trials on BM-MNCs and ۱۰ trialson MSCs) were found eligible after applying the inclusioncriteria. Both types of cells displayed significant improvementin LVEF in short-term follow-up after transplantation ofs tem cells when compared to the control group (BM-MNCs:WMD=۲.۱۳%, ۹۵% CI=۱.۲۳-۳.۰۴, p value <۰.۰۰۱; MSCs:WMD=۳.۷۱%, ۹۵% CI=۲.۳۲-۵.۰۹, p value <۰.۰۰۱) but therewas no difference between two types of cells regarding LVEFimprovement values (p value =۰.۱). Also, none of the cellsdecreased the hospitalization rate for HF significantly (BMMNCs:RR=۰.۶۴, ۹۵% CI=۰.۴۰-۱.۰۲, p value =۰.۰۵۸; MSCs:RR=۰.۹۵, ۹۵% CI=۰.۵۹-۱.۵۱, p value =۰.۸۱۳) and neither kindwere superior to another regarding rehospitalization rates (pvalue =۰.۲۶).Conclusion: The literature lacks relevant trials comparing theefficacy of BM-MNCs and MSCs in patients with acute MI.Although we found that there may not be a more efficacioustype of s tem cell in the clinical setting, this warrants future trialsdirectly comparing these two kinds of s tem cells
