Invasive Aspergillosis in post-liver transplant patients

چکیده

Background and Aim : Aspergillosis is a severe and fatal complication that causes infection intransplant recipients and patients with immunodeficiency syndrome, neutropenia, chronicgranulomatosis, and hematologic malignancies. Invasive Aspergillosis has been reported as one ofthe fungal infections with high mortality in transplant recipients. This study aimed to evaluate themanifestations of Aspergillosis fungal infections in liver transplant patientsMethods : This Descriptive cross-sectional study was conducted on Ten case of ۸۶ patients withliver transplantation who were infected with Aspergillosis fungal infections. Data were gatheredfrom the medical records of the archive of Montasryieh Hospital, Mashhad, Iran, between August۲۰۱۹ and August ۲۰۲۰.Results : In general, ۱۱.۶% (n=۱۰) of the patients who had liver transplantation from August ۲۰۱۹to August ۲۰۲۰ had been infected with Aspergillosis. Only ۶.۷% of the patients were categorizedunder the late-onset (> ۹۰ days after liver transplantation), and ۹۳.۳% of them were early-onset(< ۹۰ days after liver transplantation). Aspergillosis fungal infections were suspected on the basisof clinical or radiological signs (possible in ۳۰% of cases; n=۳). The probable diagnosis wasreported in ۶۰% (n=۶), and the proven diagnosis was observed only in one patient. Moreover, ۸۰%of the patients were diagnosed with Pulmonary Aspergillosis, and two patients had pulmonaryAspergillosis in combination with the central nervous system and cutaneous Aspergillosis. Acorrelation was reported between a comorbid disease and type of Aspergillosis (r=۰.۶۹; P=۰.۰۲).Voriconazole was effective to treat invasive Aspergillosis in all patients.Conclusion : The prevalence rate of Aspergillosis is relatively high among liver transplantrecipient populations (۱۱%). All recipients infected with Aspergillosis had at least one risk factor,including an underlying disease. It seems that therapy is effective using Voriconazole amongtransplant patients with pulmonary Aspergillosis.
