Introduction: The issue of ISBAR or delivery of patients between two shifts is one of the most critical activities and nursing activities that ensure patients' continuity of care admitted to the ward. The present study was conducted with the aim of electronic sizing in this process.Methods: The present study was performed by an action research method in Imam Reza Hospital of Mashhad. In this study, the main stakeholders were first identified. Then, with stakeholders' participation during JAD meetings in a team consisting of the head of the relevant department, nursing director, one supervisor, head and director of the hospital, and two hospitals HIS officials and a representative of the software company, the initial software design was presented. Then, during several meetings with the program's primary stakeholders, the requested corrections were implemented in the relevant software. A training booklet and face-to-face meetings were held to train the ward nursing officials.Results: The relevant team set up an electronic patient delivery and transformation system and its pilot in two selected sections as a solution. After receiving initial feedback from users, 7 out of 12 requests requested by users were applied. In this system, fields such as nurses list, bedside patients list, and nursing attention were created and based on the access created for each shift manager; he was authorized to assign nurses to patients at the end of the next shift. Then he can print this page, sign it and submit it for the person's approval in charge of the next shift.Conclusion: Establishing an electronic patient delivery system between shifts is an important issue. With this software, it is possible to evaluate the mentioned issue and ensure continuity in inpatient care, and more control can be applied to changing patients.
